The information entered in this form will be used to prepare an Enduring Power of Attorney which complies with Victorian law. As long as this document is prepared and witnessed in accordance with Victorian law, current legislation allows this document to be recognised in all other States and Territories of Australia except Western Australia, where recognition must be sought separately. This means that unless you reside in Western Australia or require your Enduring Power of Attorney to have effect in Western Australia, you can use this form to create a valid and binding Enduring Power of Attorney.
